I cant get a decent picture to help because of my skid plate. With the clutch pedal at brake pedal height. Adjust so the actuator arm is roughly 4.5" from the cable mount back at the trans. You may have to remove the actuator arm and re-install for better adjustment. This could be the issue if the actuator arm on the trans was not located correctly. Also would explain why it has never been right.

The nut that you tightened has a cam area in the end next to the yolk cross bar. But if the slack in the cable is to much than the cam on the nut can turn and back off. You should have all the slack out of the cable to where it can't turn on its own and have at least some pressure on the yolk cross bar.
